7816990: The runtime adapter forwards the request's X-Revenexx-Audit-Note to the data plane.

A client may say what a change is and why in X-Revenexx-Audit-Note, URI-encoded JSON {"title":"…","reason":"…"} (the Cockpit sends it when an operator rolls a record back). The gateway passes it to the App, and the runtime adapter now passes it on unchanged with every PostgREST request, next to X-Revenexx-Audit, so Baseline stores it with the record-history entry. The SDK never reads it and it never changes who is acting. A request without one, or with one longer than 8192 bytes or not one line of printable ASCII, sends no such header and works as before. An App describing its own change passes auditNote to createClient / createRuntimeAdapter (an empty string sends none); the remote adapter takes it as auditNote. c.actor is unchanged.
